We currently are accepting the following insurance providers:

If uncertain, please call our office to confirm whether we accept your insurance or to inquire about our self pay fee schedule.
**We do not accept Medicaid**
If you are unable to keep an appointment, please call us to cancel at least 24 hours in advance. We are a small practice and no shows can significantly impact our physicians’ schedules and their availability to provide care for other patients. We appreciate your understanding and cooperation.
Prior to your Appointment
You may print and fill out new patient forms online before your visit.
If you have any pertinent medical records that you wish to share with our office, please print and complete the Authorization to Release Medical Information Form so that we may request your medical records to be faxed over and be available to review during your consultation visit.
Date of Visit
Please arrive 15 minutes prior to your appointment time.

Driver’s License
Insurance card
Completed new patient forms
Co-pays will be collected per your insurance policy.
Laboratory Testing
Our office staff will help you arrange for all studies and/or labs orders.
We do not take blood samples at our clinic. For all lab orders, you will be directed to one of several LabCorp offices to get blood drawn.
Consultation
For new patients, have ready a list of your current medications including herbs/supplements along with the dosage and frequency of use.
For follow up patients, make a note of any changes to your medications, herbs/supplements (i.e. change in dose, stopped or started any new medications)
